What Causes Long COVID?

Research is ongoing, but several mechanisms have been identified:

Persistent viral reservoirs: Fragments of SARS-CoV-2 or active viral particles may persist in tissues, maintaining chronic immune activation.

Autoimmunity: COVID-19 can trigger autoantibody production, and many Long COVID patients develop autoimmune-like responses.

Chronic inflammation: Sustained elevation of inflammatory cytokines and markers is common in Long COVID patients.

Endothelial dysfunction and microclotting: Damage to blood vessel linings and the formation of microclots can impair oxygen delivery to tissues, contributing to fatigue and cognitive symptoms.

Mitochondrial dysfunction: Impaired cellular energy production may explain the profound fatigue and exercise intolerance.

Autonomic nervous system dysfunction: Dysautonomia, including POTS (Postural Orthostatic Tachycardia Syndrome), is frequently observed after COVID.

Our Diagnostic Approach

I conduct a thorough assessment including detailed symptom mapping and timeline, comprehensive blood work (inflammatory markers, autoantibodies, thyroid function, iron studies, vitamin D, B12), cardiovascular assessment (ECG, sometimes echocardiography or Holter monitoring), assessment for orthostatic intolerance, pulmonary function evaluation, and gut health assessment.
