Estrogen Dominance – Symptoms, Causes & Treatment in Zurich

When Oestrogen Takes Over – A Hidden Hormonal Imbalance

Heavy, painful periods. Breast tenderness that lasts for weeks. Mood swings, water retention, weight gain around the hips, and a feeling that your hormones are completely out of control. If this resonates with you, you may be experiencing oestrogen dominance – a condition where oestrogen levels are disproportionately high relative to progesterone.

At our practice in Zürich Seefeld, I see oestrogen dominance frequently, particularly in women in their 30s and 40s approaching perimenopause. It is also increasingly common in younger women due to environmental and lifestyle factors. This is a condition that conventional medicine often overlooks, yet it significantly impacts quality of life and long-term health.

What Is Oestrogen Dominance?

Oestrogen dominance does not necessarily mean your absolute oestrogen levels are too high – although they can be. More often, it means the ratio of oestrogen to progesterone is skewed. This can occur because oestrogen is genuinely elevated, because progesterone is too low (common in anovulatory cycles and perimenopause), or because oestrogen is not being properly metabolised and eliminated.

Common Causes

Anovulatory cycles: Without ovulation, no corpus luteum forms, and progesterone production drops dramatically. This is common in PCOS, perimenopause, and during periods of high stress.

Perimenopause: In the years before menopause, progesterone production declines faster than oestrogen, creating a relative excess of oestrogen even as both hormones eventually fall.

Xenoestrogens: Environmental chemicals that mimic oestrogen – found in plastics (BPA), pesticides, cosmetics, and household products – add to the body’s oestrogen load.

Poor oestrogen metabolism: The liver detoxifies oestrogen through specific metabolic pathways. If liver function is compromised, or if the gut microbiome is not properly eliminating oestrogen metabolites (via the estrobolome), oestrogen recirculates.

Excess body fat: Adipose tissue produces oestrogen via aromatase activity. Higher body fat means higher oestrogen production.

Chronic stress: Cortisol competes with progesterone for precursor hormones and can also impair oestrogen detoxification pathways.

Symptoms of Oestrogen Dominance

The symptom profile is wide-ranging: heavy and painful menstrual bleeding, breast tenderness and fibrocystic changes, PMS with mood swings, irritability, and anxiety, water retention and bloating, weight gain particularly around hips and thighs, fibroids and endometriosis, headaches or migraines (often cyclical), fatigue and brain fog, and difficulty sleeping. In men, oestrogen dominance can manifest as gynaecomastia, weight gain, and reduced libido.

How We Investigate

I assess hormones at the right time in your cycle – typically day 19-21 for progesterone, and day 3-5 for oestradiol, FSH, and LH. I also evaluate oestrogen metabolism through specific urine or blood tests that show which metabolic pathways are active. Liver function, thyroid status, and key nutrients (B vitamins, magnesium, DIM/I3C precursors) are all part of the picture.

What We Do: Restoring Hormonal Balance

Support progesterone production: This may involve addressing anovulation, managing stress, and using targeted nutrients like vitamin B6, zinc, and vitex (chaste tree berry) to support the corpus luteum.

Optimise oestrogen detoxification: Supporting liver Phase I and Phase II detoxification with cruciferous vegetables, DIM (diindolylmethane), calcium-D-glucarate, and B vitamins helps the body metabolise and eliminate excess oestrogen.

Heal the gut: A healthy gut microbiome with proper beta-glucuronidase activity ensures that oestrogen metabolites are excreted rather than reabsorbed. Probiotics, fibre, and gut barrier support are key.

Reduce xenoestrogen exposure: Practical guidance on reducing environmental oestrogen exposure – switching to glass containers, choosing clean personal care products, eating organic where it matters most.

Bioidentical progesterone: When indicated, bioidentical progesterone supplementation can restore the oestrogen-progesterone ratio effectively and safely.

Conclusion

Oestrogen dominance is a common, treatable hormonal imbalance that affects both women and men. Rather than simply suppressing symptoms with hormonal contraceptives or anti-inflammatories, a root-cause approach can restore balance and improve your quality of life. If you suspect oestrogen dominance is affecting your health, I invite you to book a consultation at our practice in Zürich Seefeld.
