Cortisol and stress: What happens when your adrenals are overwhelmed

Chronic stress is the modern epidemic. Cortisol is our stress hormone, and with constant overload, the adrenals lose their flexibility. This leads to a pattern that some call "adrenal fatigue". In my Zurich practice, I see this daily — people who are burnt out, feel constantly tired, and cannot recover.

The normal cortisol pattern and what stress does to it

Cortisol should be high in the morning (this wakes us up), then gradually fall during the day. At night it should be low so we can sleep. Chronic stress often inverts this curve. Some patients have low morning cortisol and are unable to wake up. Others have elevated cortisol at night and cannot sleep.

The symptoms of cortisol dysregulation

Extreme fatigue despite sufficient sleep, sleep problems, memory problems, weight gain especially around the belly, constant susceptibility to infections, muscle weakness — these are the classic symptoms. Also a flat emotional response, lack of motivation, even depression can follow. This is not just "burnout" — this is a measurable hormone disorder.

The saliva test: The right diagnostic tool

Blood cortisol is less reliable than saliva cortisol. The saliva test shows the free, biologically active cortisol. I measure saliva four times daily: morning, midday, afternoon, and before bedtime. This way I see the full pattern. This gives me and my patients the complete picture to plan targeted measures.

Why standard blood tests aren't enough

A single blood cortisol test at one time of day is almost useless. The value fluctuates constantly. It's like measuring the temperature on a hot day at 2 p.m. and assuming that's the average daily temperature. That doesn't work. The saliva day profile shows the real issue.

Treatment: Stress management is not enough

Sure, meditation and yoga help. But if the adrenals are really exhausted, that's not enough. I work with adaptogens like rhodiola, ashwagandha. Magnesium is essential. Nutrient deficiencies are often a problem with stress. Also getting good sleep is necessary — that's when adrenal regeneration happens.

Burnout prevention and recovery

I see patients who have completely broken out of the hamster wheel. That was necessary. That's the only way to true recovery. Medically we support this with functional medicine — nutrient optimisation, sleep hygiene, nervous system regulation. True recovery takes time and medical support.
